Position Summary:
We are seeking a dedicated and clinically competent Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to join our team. Reporting directly to the Director of Nursing, the LPN/LVN is responsible for providing high-quality patient care using the nursing process—assessment, planning, intervention, implementation, and evaluation. This role requires effective interaction with patients, their significant others, and other healthcare team members while maintaining professional nursing standards.

The LPN has primary responsibility for providing medical services, counseling, supervision, and program plan implementation to clients during scheduled shifts within their scope of practice.

Key Responsibilities:

  • Assessment, Planning, and Implementation:
  • Conduct and document patient assessments including current health status, medical history, and ongoing evaluations.
  • Develop and implement a plan of care based on patient needs, including physical, psychosocial, and environmental factors.
  • Prioritize nursing actions and formulate care plans based on nursing practice standards.
  • Continuously update the plan of care based on new data and patient condition.
  • Patient Care & Medication Administration:
  • Administer medications according to established protocols and document patient responses to treatments.
  • Perform direct patient care, ensuring comfort and well-being while adhering to policies and procedures.
  • Administer IV treatments (if certified and within the scope of practice).
  • Documentation & Communication:
  • Document all patient care activities, interventions, and patient responses accurately and consistently.
  • Communicate daily with physicians and other team members to discuss patient needs, assessments, and progress.
  • Provide health education to patients and their families, ensuring they understand instructions and have the necessary resources.
  • Emergency & Technical Skills:
  • Demonstrate appropriate technical skills when performing treatments and procedures per physician orders.
  • Handle emergency situations promptly, with professionalism and precision.
  • Environmental & Safety Standards:
  • Maintain a clean, safe, and organized environment for patients and staff.
  • Ensure all equipment is properly stored, maintained, and cleaned according to safety standards.
  • Adhere to infection control policies and report any safety hazards or equipment malfunctions.
  • Teamwork & Training:
  • Collaborate effectively with team members and assist in the orientation and training of new staff.
  • Volunteer to fill open shifts as needed and contribute to team cohesion.
  • Participate in ongoing education and training, including mandatory skill updates and meetings.
  • Problem Solving & Efficiency:
  • Exercise sound judgment in addressing concerns or unusual situations.
  • Organize tasks and workflows to maximize productivity and meet patient needs efficiently.
  • Follow up on unusual orders or service requests to ensure proper care is delivered.

Minimum Qualifications:

  • Graduate of an accredited school of Nursing.
  • Valid licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state (or valid compact licensure).
  • Current CPR certification from the American Heart Association or American Red Cross.
  • Previous experience in a patient care setting preferred.
  • Knowledge of the nursing process and its application in providing patient care.
