Job Title: Qualified Health Home Specialist
Position Overview: As a Qualified Health Home Specialist within the Behavioral Health Home (BHH) Services, you will play a crucial role in supporting and assisting clients in reaching their health and wellness goals. With a focus on building trusting relationships and addressing barriers to care, you will serve as a coach, providing ongoing motivation, encouragement, and support. Your responsibilities will include health and wellness promotion, individual and family support services, care coordination, and facilitating access to community resources.
Roles and Responsibilities:
- Client Support and Coaching:
- Interact regularly with clients, either in person or via phone, to establish trusting relationships and assist in goal setting.
- Help clients identify barriers to accessing care and develop strategies to overcome them.
- Provide ongoing motivation, encouragement, and positive feedback to clients as they make progress towards their health and wellness goals.
- Health Promotion and Care Coordination:
- Serve as the primary entity responsible for health and wellness promotion services within the BHH program.
- Assist the BHH Systems Navigator in coordinating care, serving as the secondary entity responsible for care coordination services.
- Meet with clients before and after appointments to help organize health concerns, review provider instructions, and ensure understanding and access to necessary resources.
- Individual and Family Support Services:
- Provide regular individual and family support services to address clients' overall needs and enhance their well-being.
- Offer guidance, education, and resources to empower clients and their families in self-management and decision-making.
- Transitional Care and Referral Services:
- Contribute to the provision of comprehensive transitional care services, facilitating smooth transitions between care settings.
- Assist clients in accessing community and social support services, coordinating referrals as needed to address their holistic needs.
- Wellness Initiatives:
- Conduct wellness groups and activities as appropriate, leveraging individual training and expertise to promote holistic health and well-being.
- Collaborate with the BHH team to develop and implement wellness initiatives tailored to the needs of the client population.
Qualifications:
- Community health worker as defined in M.S. 256B.0625 Subd. 49.
- Peer support specialist as defined in M.S. 256B.0615.
- Family peer support specialist (pending certification at DHS).
- Case management associate as defined in M.S. 245.462 Subd.4 (g) or M.S. 245.4871 Subd. 4 (j).
- Mental health rehabilitation worker as defined in M.S. 256B.0623 Subd. 5 (4).
- Experience working in a healthcare or social services setting, preferably in a role involving client support and coordination of care.
- Strong communication, interpersonal, and coaching skills, with a compassionate and client-centered approach.
- Knowledge of community resources, social support services, and health promotion strategies.

Pay: $23.00 - $27.00 per hour
