Specialist

The Rehabilitation Specialist is in charge of conducting and preparing rehabilitation assessments for clients, assisting PRP specialists, and reviewing PRP progress notes to ensure their completion.

The Rehabilitation Specialist helps to build and analyze tailored rehabilitation plans to fulfill the needs of each consumer in line with state standards.

This involves implementation and documentation in accordance with COMAR guidelines. This job reports to the Director.

Educate PRP Workers on how to deliver rehabilitation treatments that align with the goals outlined in the individual's IRP.

Primary Tasks and Duties

Screen referrals, schedule, and perform rehabilitation assessments. Review and manage the collecting of necessary information about the referred client's background, social history, and previous therapeutic interventions in order to create rehabilitation programs for them.

Provide quality evaluation of notes and monthly summaries, verifying goals work as indicated in IRPs.

Ensure that notes are prepared in accordance with applicable federal and state legislation, rules, policies, and procedures.

Rehab Specialists will manage a caseload of around 15 clients and make at least three visits each client per month.
