OVERALL RESPONSIBILITY :

Case Management and Care Coordination
- An Initial Assessment that assesses the clients functional limitations resource availability and identified community supports to determine the case management interventions delivered within ESHs Outpatient Clinic.
- Care Coordination shall consist of the following interventions as deemed appropriate for the clients needs :
Assistance in the development and implementation of a plan for accessing benefits and entitlements to include the completion of a Medicaid application (if not already enrolled) and Social Security Insurance (SSI) application when appropriate.
The program case management will work with ESHs fiscal unit to assist in the verification of any active medical coverage and eligibility for state programs to include Medicaid SSI and any state waiver services
In the instance in which the client is not receiving mental health or substance abuse services the client will be referred to a Community Mental Health Center or Substance Use provider as well as provide
ongoing consultation with the provider to ensuring the client is receiving adequate services to meet their identified needs
In the instance in which the client is diagnosed with an Intellectual or Developmental Disability and is not active with the Division of Developmental Disabilities the case manager will assist the client with the application process and serve as a liaison to the Division.

Masters Degree in social work or closely related field is required.
Position is for 1 year possible to be extended.
