A healthcare system in southern Vermont, near the New Hampshire border, is seeking a skilled Nurse Practitioner or Physician Assistant to join their team and provide high-quality care to residents of their affiliated Skilled Nursing Facilities.
Key Responsibilities
- Provide comprehensive medical services, including prescription medication, nutrition planning, exercise therapy, and traditional nursing treatments, across multiple Skilled Nursing Facilities and hospital settings.
- Collaborate with Family Practice Physicians to deliver exceptional patient care.
- Work collaboratively as part of an interdisciplinary team to ensure coordinated care.
Requirements
- Board Certification as a Nurse Practitioner (NP) or Physician Assistant (PA).
- At least 2 years of experience in post-acute care or Skilled Nursing Facility settings (strongly preferred).
- Valid DEA License.
