- BC Family Medicine or Internal Medicine
- Hospice and Palliative medicine or Geriatrics fellowship is a plus but not required
- Active NY license, BLS
- Local providers required
- Hospice home care setting
- Available ASAP for ongoing need
- Mon - Fri; 8a-4 : 30p
- Call coverage; 1 week night per week from 4pm-8am and 1 in 4 / 5 weekends from 4p on Fri until 8a on Mon
- EMR used is EPIC
- Starting hourly rate $130 / hr pending experience and open to discussion for the right candidate
