Description

Organization Overview: The Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Neighborhood Center (Isaacs Center) are well-established Manhattan community-based agencies that support services to approximately 30,000 New Yorkers. We are two of New York City’s leading human services organizations meeting the needs of children, youth and families, homeless individuals, and older adults through-out Manhattan and New York City. The Stanley M. Isaacs Neighborhood Center (Isaacs Center) is a non-profit, multi-service organization focusing primarily on the needs of children and low-income families, out-of-school and out-of-work youth, and aging New Yorkers. We operate at community centers located in the neighborhoods of East Harlem and Yorkville and seek to deliver programs that are impactful, innovative, and intergenerational.

 

Position Summary: The Program Coordinator (Taft) is responsible for managing the day-to-day operations of the Community Telehealth Project. The Program Coordinator (Taft) will also be providing case management services to older adults and leading program activities. Other duties will include conducting outreach to promote programs to identify and enroll new participants and working with partner agencies to meet deliverables.  Provision of services will be in accordance with Isaacs Center policies, contractual guidelines, and standards.   

 

Schedule: 35 hours per week.

 

Salary: $45,940- $57,425

 

 

Roles, Responsibilities, and Essential Duties


  • Manage the day-to-day operations of the Community Telehealth Project which include: developing creative methods to promote the program, identifying and enrolling participants, acting as a liaison with funders and community partners 

  • Provide case management services to older adults residing within the program catchment area and surrounding community which include: applying for government benefits and entitlements, advocating for the clients’ medical, mental health, housing-related and other needs, and coordination of care  

  • Prepare reports and documentation as stipulated by contractual and agency standards and procedures 

  • Assist with NDA Senior Service Program and provide staff coverage when needed  

  • Assist with program planning which include: developing, implementing, and promoting educational/recreational groups, and leading programs 

  • Develop and foster relationships with community and government agencies   

  • Other duties as assigned 

 

Qualifications:

The successful candidate for the role of Aging Services Coordinator should have:

  • Demonstrated experience in program startup
  • Ability to multitask and manage priorities, developing and leading programs, and providing thoughtful leadership.
  • Excellent interpersonal, communication, time management, organizational, and teaming skills are essential.
  • Bilingual English/Spanish fluency is required.

 

Education/Skills/Experience:

  • Bachelor’s Degree required 

  • Bilingual English/Spanish fluency required 

  • Must be comfortable using Microsoft Office products and have the ability to learn multiple databases for documentation purposes 

  • Previous experience working with older adults strongly preferred 

  • Ability to be open and flexible to a start-up environment strongly preferred 

  • Ability to handle confidential information with discretion 

  • Ability to work non-traditional hours (e.g. evenings, weekends, holidays) required

 

Physical Requirements 

  • Ability to move around between rooms  

  • Ability to set up rooms with tables and chairs 

  • Ability to walk and/or stand frequently 

 

Working Conditions 

  • Older Adult Center environment located in the New York City Housing Authority Taft Development  

  • Open workspace 

  • Tabling at community events e.g. health fairs, tenant meetings to conduct outreach 

  • Home visits as needed
