Golden Gate Hospice Care is seeking a compassionate, organized, and dedicated Patient Care Coordinator to serve as a vital link between patients, families, and the care team. In this role, you will ensure patients receive seamless, compassionate end-of-life care by managing care plans, facilitating communication, and coordinating services. Key Responsibilities: Patient and Family Coordination:
- Act as the primary point of contact for patients, families, and caregivers, providing emotional support and clear communication.
- Coordinate initial patient assessments and admissions alongside the clinical team.
- Serve as a liaison between families and healthcare providers to ensure alignment in care decisions.
- Answer and direct office phone calls in accordance with the phone tree.
- Follow up biweekly with families to confirm medication and supply deliveries or as changes occur.
Care Management:
- Develop and update individualized patient care plans with the interdisciplinary team.
- Monitor patient progress and escalate care needs as necessary.
- Track nursing visits and flag any delinquent visits or missing notes.
- Schedule and coordinate care services, therapies, and equipment deliveries.
- Communicate patient and nursing updates to the Director of Nursing and Administrative Medical Director.
Administrative Duties:
- Maintain accurate, timely patient records following state regulations and company policies.
- Verify insurance coverage and obtain required authorizations for hospice services.
- Manage patient consents to ensure completeness and compliance.
- Participate in interdisciplinary team (IDT) meetings and assist administrative staff as needed.
Compliance and Education:
- Ensure adherence to California hospice care regulations and standards.
- Educate families on hospice services, end-of-life care, and available support resources.
- Provide after-hours phone/text support as necessary.
Qualifications: Required:
- High school diploma or equivalent (Bachelor's degree preferred).
- Minimum 1 year of experience in healthcare coordination, patient care, or a related field.
- Understanding of hospice care principles, medical terminology, and California healthcare regulations.
- Strong communication, organization, and interpersonal skills.
Preferred:
- Experience with Electronic Medical Records (EMR) systems.
- Spanish-speaking ability highly desirable.
