Golden State Urology offices are located in Stockton, Sacramento and Folsom and offers a wide range of services for men and women. Our physicians are dedicated to excellent patient care and experts in the latest care techniques. Our staff is knowledgeable and helpful, and our offices are friendly and warm. We are looking for a Professional Registered Nurse Practitioner to join our team and provide comprehensive care coordination for a disease-specific group of urology patients. The candidate would be spearheading the creation of programs and patient pathways and provide individual assistance to patients, families and caregivers to help identify and overcome barriers which may hinder quality medical care.
Job Requirements:
- Bachelor of Science degree in Nursing preferred
- Master’s Degree, post Master’s Certificate or Doctoral degree in Nursing
- Licensed in practice’s state as a Registered Nurse
- Minimum 3 years RN experience in medical/surgical nursing preferred
- Effective communication and writing skills
- Ability to multitask and wok in a fast paced environment
- Must possess strong customer service and problem solving skills
General Responsibilities:

- Attends staff meetings, clinical meetings and training in-services as scheduled and required

- Report in good faith to the unit manager or practice administrator any suspected violation of applicable laws and/or regulations

- Performs other duties as assigned by manager or physician while maintaining a helpful attitude
- Strong oral and written communication skills for corresponding with physicians, patients and fellow employees
Duties & Responsibilities
- Spearheading the creation of programs and patient pathways
- Manage the case load of patients with chronic disease under the care of our physicians
- Serve as a focal point for the patients, directing them to the appropriate resources compassionately and in a timely manner
- Assist patients with understanding the disease process, treatment options and through consultation planning, assist them with optimizing visits with their physician
- Ensure knowledge of resources and treatment options are consistently up to date
- Improve healthcare access for patients and promote client knowledge
- Collaborate with physicians to determine care needed at discharge from a hospital setting
- Consult with department staff to eliminate barriers to efficient delivery of care
- Assess the patient’s plan of care and develop, implement, monitor and document the utilization of resources and progress of the patient through their care
- Serve as a liaison for referring physicians and assist with scheduling initial tests and consultations
- Monitor data to identify patients to ensure full continuum of care.
- Ensure that the patient and family understand the diagnostic process, care strategy and recommended actions and are provided with appropriate information in coordination with the physician
- Respond to patient requests for information regarding the disease process, expected side effects and community resources
- Establish and promote a collaborative relationship with physicians, payors and other members of the health care team
- Spearhead clinical trial studies
- Participate in community outreach
- Able to commute to all three clinic locations to provide patient care.

Job Type: Full-time
Pay: $125,000.00 - $165,000.00 per year
