R.N. - Weekend Option

Job Description:
Performs services reasonable and necessary for treatment of an illness or injury in the home setting. Services include observation and assessment, teaching and training, management of a care plan, and assistance with medications and treatments. RN will have an assigned patient caseload with planning, scheduling, and documentation responsibilities and will provide oversight and care to any patients needing skilled nursing services within our Home Health coverage region. Treats patients with all types of disabilities and multiple medical problems. All applicants will be expected to sign a one-year commitment to taking on the Weekend Option position, in which the nurse will work a 12-hour shift on Saturday and a 12-hour shift on Sunday, earning six bonus hours per 12-hour shift completed, for a total of 36 hours/weekend. Weekend differential and holiday pay apply only to hours worked. Staff members may take up to two weekends off/year.

Responsibilities:
Assume primary responsibility for a patient/family caseload, Develops and evaluates the plan of care, Initiate communication with attending physicians, other hospice staff members and other agencies, Maintain regular communication with the attending physician concerning patient/family caseload, Obtain data on physical, psychological, social, and spiritual factors that may influence patient/family health status, Maintain up-to-date patient records so that problems, plans, actions, and goals are accurately and clearly stated, Seek input from other team members regarding the patient plan of care to obtain additional knowledge and support.

Education/Credentials:
Graduate of an accredited school of nursing.
Current Missouri RN license.
BLS certified.
Current MO driver's license and car insurance.
Registration with Missouri Family Care Safety Registry with no findings.
One year experience.
Home Health and/or Hospice experience desired.

Starting Range: $31.28-39.10
