CARE COORDINATOR FOR ADDICTION RECOVERY
Goochland Powhatan Community Services Board is seeking a Care Coordinator for Addiction Recovery to join our team. As a key member of our multidisciplinary team, you will work closely with clients, family members, and healthcare providers to develop and implement personalized plans for recovery.
Your primary responsibilities will include completing comprehensive psychosocial assessments, creating individualized service plans, monitoring progress, and ensuring seamless coordination of care. You will also collaborate with community partners to identify and connect clients with relevant resources and support services.
To succeed in this role, you must possess a deep understanding of addiction recovery principles and practices, as well as excellent communication and interpersonal skills. A Bachelor's degree and relevant experience are required. CSAC certification is highly desirable, and familiarity with MAT for opioid dependence is an asset.
