Good Deeds Case Management Services provides care coordination for clients and families who desire to maintain a good quality of life. We provide assessment, service planning, and service monitoring to participants enrolled in the Community Choices waiver. We’re seeking an experienced case manager to join our team as we set the standard for a participant-focused approach. The ideal candidate will share our passion for improving people’s lives by providing access to the best care possible. The role demands compassion, and exceptional interpersonal, communication, and time management skills.
Responsibilities
- Manage case assignments, develop service plans, track progress, and execute case closures
- Advocate for clients with advice and inform clients of services offered through the Community Long-Term Care
- Monitor the client's progress monthly
- Conduct in-home visits with clients and families
- Assess and act accordingly in response to treatment barriers and psychosocial issues
- Conduct assessments and staff with the appropriate state staff
Required skills and qualifications
Bachelor’s degree or Master’s degree with at least two years of assessment and care planning experience with clients

- Excellent interpersonal, verbal, and written communication skills
- Superb critical-thinking, organizational, and analytical skills
- Ability to work with all age groups and populations
Preferred skills and qualifications
- Prior CLTC experience is preferred
Job Type: Part-time
Pay: $23,500.00 - $26,000.00 per year
Schedule:
- Monday to Friday
