Job Overview
The Home Maker Multipurpose Task role is essential in providing compassionate and dedicated support to individuals community care setting. This position focuses on enhancing the quality of life for the elderly , particularly those with developmental disabilities, by assisting with daily living activities and promoting their independence. This position involve every other week coverage that must be 8 hours, The tasks to be assigned and include the following, WELLNESS CARE, CHECKING TO ENSURE ALL CLIENTS ARE BEING CARED FOR VERY WELL, AND COVERING FOLLOWING THE PLAN OF CARE, KEEPING PLAN OF CARE AND ENSURING THAT ALL CLIENTS ARE COVERED DURING HOLIDAYS AND WEEKENDS, ALL CALL OFFS MUST BE COVERED, ALL CLIENTS ARE TRACKED TO ENSURE THAT ALL SCHEDULD VISISTS ARE COVERED AT 100 PERCENT THAT MEANS ALL SCHEDULD VISIT ARE SEEN. THE STAF WILL COVER AND PERFORM ALL SUPERVISORY VISITS, ALL FIRST AND NEW ADDMIISSION, THEY HAVE TO PRESCREEN THE PRIVATE CLIENTS AND THEIR HOURS, COLLECT ALL THE REQUIRE DOCUMENT, CREAT THE PLAN OF CARE, INITAL THE CONTRACT AND COLLECT , THE FIRST PAYMENT AND POST ADD AND SET UP THE FIRST VISIT ALL TO BE SCANED IN AXIS CARE, PERFORM VISIT VERIFICATION , SCREEN PRIVATE CLIENT AND ENCOURAGE THEM FOR THEIR READYINESS TO START SERIVICES. The ideal candidate will possess a strong understanding of home care rights and patient care, ensuring a safe and nurturing environment.demografic areas, must be aggressive , adequate knowledge of computer, willing to work every other weekend. able to drive, mush have good driving record which will be pulled from the secretary of start to be allowed to use the company car.
Responsibilities
- Able to conduct visit as schedule for new admission, call offs.
- Perform supervisory visits as assigned.(withIDOA TRAINING)
- Able to develop plan of care.
- willing to work every other weekend, and some holidays
- Must be able to perform visit verification
- Willing to answer calls.
- Must have skill and caregiver experience.
- Must be willing to drive the companies car.
Requirements
- Experience in home community care is preferred.
- Knowledge of working with individuals with developmental disabilities is highly desirable.
- Familiarity with patient observation techniques and patient care best practices.
- Understanding of home care rights and advocacy principles within the clients living environments.
- Strong communication skills to effectively interact with residents, families, and team members.
- Ability to work collaboratively within a multidisciplinary team while demonstrating empathy and patience.
- A commitment to maintaining a safe, clean, and supportive living environment for all clients
- Must have GED and or diploma
- Must be Caring and accurate with time, and attention to details.
- fluent in English and Spanish a must.
