Good Neighbor Health Clinic is seeking a dedicated Patient Care Coordinator to join our team. Responsibilities:
The Patient Care Coordinator plays a vital role in supporting clinical staff and volunteers, ensuring continuity of care, and addressing specific patient needs while upholding the mission of Good Neighbor Health Clinic. Key responsibilities include:
- Coordinating the hand-off of patients to team nurses and other community resources following intake.
- Serving as a patient advocate by supporting medical and nursing volunteers and staff with follow-up and post-appointment care.
- Recognizing urgent situations and collaborating with the Medical Clinic Team to ensure continuity of care, appropriate referrals, and necessary resources.
- Maintaining accurate and up-to-date patient records and submitting data as required.
- Managing post-encounter casework, ensuring timely follow-up.
- Contributing to the development of patient care standards and working with community programs to provide patient education and resource coordination.
Qualifications:
- Current licensure as an LPN, RN, or EMT currently licensed in Vermont and New Hampshire.
- Basic Life Support (BLS) Certification.
- Minimum of three years of clinical experience, with at least 1-2 years of public health exposure.
- Strong team player, dedicated to working collaboratively with colleagues and volunteers to fulfill our mission.
- Excellent written and verbal communication skills, with the ability to document patient information clearly and consistently.
- Exceptional organizational skills and attention to detail.
- Commitment to patient confidentiality and accurate data management.
- Ability to prioritize tasks and work independently with minimal supervision.
