What are the responsibilities and job description for the Hospice LPN Wexford IPU position at Good Samaritan Hospice?
POSITION SUMMARY
The Licensed Practical Nurse (LPN) works under the guidance of the RN Charge Nurse to give direct nursing care in the Cabot inpatient hospice facility in accordance with the individualized plan of care developed by the patient and/or family, physician and the hospice team.
This is a part time position working (2) - 12 hour shifts (one daylight and one night shift) and includes a weekend and holiday rotation.
QUALIFICATIONS
1. Implements and evaluates all skilled and technical patient/family care activities in accordance with the individualized plan of as directed by the RN member of the Interdisciplinary Group (IDG)
2. Conducts a physical examination and observation of the patient on each shift and as necessary, and reports changes in the patient’s condition to the RN member of the IDG team
3. Participates in patient and/or family teaching activities as outlined by the plan of care
4. Together with the hospice team, provides care as established by the IDG to meet the physical, emotional and spiritual needs of the patient as necessary, and appropriately supports the family at the time of death
5. Provides personal care such as (but not limited to): bathing, grooming, dressing, shaving, feeding; checking vital signs; assisting with ambulation; changing position in bed; assisting with transfers, changing bed linens; assisting with elimination; providing oral hygiene, skin care, foot care, ear care, light house cleaning and meal preparation
6. Completes inpatient facility housekeeping duties as necessary
7. Completes all computer and/or paper documentation requirements within the timeframes specified
8. Maintains regular communication and interaction with patients, families, physicians and hospice team members
9. Works shifts as needed, including weekend rotations; participates in required on-call responsibilities as assigned

QUALIFICATIONS
- Current LPN license in the state of Pennsylvania
- Minimum of two [2] years experience in patient care preferred
- Demonstrates excellent clinical skills
- Current CPR certification
- Solid computer skills, including Word
- Excellent organizational skills with attention to detail
- Outstanding communication skills (both written and verbal)
