We are currently seeking a FT LPN – Palliative Care for our Roanoke based service area.
As the LPN in our Palliative Care Department, you will be responsible for the provision of nursing care to patients according to an established physician plan of care, with service delivery under the direction of The Medical Director and in compliance with applicable state Nurse Practice regulations and Good Samaritan policy and procedure. You will assist the provider with coordination of care, schedule patient visits as needed, and conduct follow up and phone calls to patients.
