We are currently seeking a Nurse Practitioner for our service areas.
As a Nurse Practitioner, works in collaboration with Good Samaritan Hospice staff to provide comprehensive, quality palliative care services to patients under the direction of the Hospice Medical Director. The NP is responsible for providing patient visits for palliative care patients both new and established and coordinating their plan of care with other designated staff. The NP will fulfill the requirements of the “face-to-face encounter” with a Medicare beneficiary, as needed. Patient visit will include visits to patient’s homes, facilities and/or hospitals. In addition, the NP will engage in independent decision-making about a patient’s healthcare concerns and provide palliative care services on an as needed basis
Schedule: Monday through Friday, 8:30 am to 5 pm, but please note that hours outside of these may require a call on some evenings and weekends.
