Duties & Responsibilities :
- Conducts initial spiritual assessments to identify spiritual needs of our patients and family members and incorporate this into the individualized plan of care.
- Provides effective interventions on initial and subsequent visits to meet the unique goals of our patients and family members.
- Provides outreach and education / support to facilities and other community resources
- Maintains relationships with local spiritual / faith providers and resources
- Serves as spiritual support for staff regarding issues of grief and loss as it related to their professional roles
- Participates in Ethics and other relevant committees to enhance our overall program
- participates in the ongoing development of the role of Chaplain within the agency and within community facilities
