The Weekend Admissions/Liaison introduction to hospice and palliative care for a patient and their family. Initiates the plan of care and discharge plan and communicates with the interdisciplinary team regarding patient goals and treatment plans. Promotes all Good Samaritan programs to acute care facilities through collaborative relationships with the facility by providing opportunities for professional development and community education.
The schedule will be 10 AM- 10:30 PM on Saturday and Sunday with rotating holidays.
