Unlock your potentialJob Summary and QualificationsThe Outpatient Mental Health Therapist in our PHP and IOP programs deliver exceptional, patient-focused care by providing clinical services in a multidisciplinary team focused on psychodynamic therapy, group psychotherapy, and psycho-education to patients admitted into the Outpatient Programs.What you will do in this Role : Conduct one-on-one and group psychoeducational and process groups, focused in skills / resilience training and evidence-based treatment.Develop treatment plans and attends treatment planning meetings and reviews on an ongoing basis.Coordinate with existing outpatient therapy care teams.Assess environment and patients safety status and reports / responds to actual and potential safety issues or changes in the patients condition.Identify risk factors, screening criteria, and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement).Complete assessments, group notes, and all other documentation promptly and in compliance with requirements, as appropriate to scope.Collaborate with the multidisciplinary team to assess and coordinate patients discharge planning needs and readiness for discharge.Conduct post-discharge follow-up as indicated.What qualifications you will need : Basic Cardiac Life Support Nonviolent Crisis Intervention Masters Degree - Psychology, Counseling, or Social Work-REQUIRED3 years of therapeutic experience -Strongly PreferredPsychotherapy experience -REQUIREDAdvanced Licensure in the state of California (LMFT, LCSW, LPC, or Psychologist) -REQUIRED Good Samaritan Hospital has been serving Silicon Valley since 1965, giving patients access to highly-trained physicians and advanced technology.
