Essential Duties and Responsibilities:
The Re-Entry Discharge Planner plays a crucial role in providing re-entry services to inmates, guiding them to programs that match their needs, and ensuring they receive necessary assistance upon release.
The duties include:
- Manage an assigned caseload of inmates.
- Assess clients' re-entry planning needs and wants.
- Coordinate clients' needs and wants with their attorneys and/or public defenders.
- Develop individualized re-entry plans tailored to each client's needs and goals.
- Arrange residential program screenings and transportation.
- Confirm clients' Medi-Cal status and assist with Medi-Cal applications as needed.
- Facilitate linkage with supportive services, including drug and alcohol treatment, mental health services, homeless services, housing resources, clothing resources, and public health services.
- Coordinate with Collaborative Courts such as Restorative Court, Veterans Treatment Court, Substance Abuse Treatment Court, Mental Health Treatment Court, and Clean and Sober Drug Court.
- Coordinate aftercare services with parole agents and the Division of Parole Operations.
- Secure placements in transitional living programs as appropriate.
- Prepare narrative and statistical reports, documents, and correspondence related to re-entry planning services.
- Participate in multi-disciplinary team meetings within the Sheriff's Office and with community partners.
- Maintain written records of all contacts and case progress notes.
- Document any obstacles encountered in achieving the goals stated in the inmate's individualized plan.
- Promptly notify the manager of any work orders or supply needs.
- Attend staff meetings, complete training timely, and accurately complete timecards.
- Perform related duties as assigned by the supervisor.
- Maintain compliance with all company policies and procedures.
Education and/or Work Experience Requirements:

- High School Diploma or equivalent: A minimum educational requirement to demonstrate basic literacy and communication skills.
- Bachelor’s Degree in Criminal Justice, Human Services, or Social Work Required.
- One year of social service case worker with experience in working with homeless persons, or equivalent.
- Experience in case management practices and working knowledge of various appropriate counseling techniques
- Excellent communication skills: Ability to effectively communicate with shelter residents, colleagues, and external stakeholders.
- Crisis intervention skills: Capacity to handle crises and de-escalate conflicts.
- Empathy and compassion: Demonstrating genuine care and understanding for individuals experiencing homelessness.
- Ability to work in a team: Collaboration and cooperation with other shelter staff, volunteers, and external agencies.
- Excellent computer proficiency (MS Office – Word, Excel, and Google)
Required Qualifications:
Employment Eligibility Verification:

Negative Tuberculosis Test:
- Must provide documentation of a recent negative tuberculosis (TB) test result to ensure a safe and healthy work environment.
Background Screening:
- Successful completion of a comprehensive background screening, which may include criminal record checks, employment history verification, and reference checks.
- Must also meet the requirement of passing the Department of Justice background in accordance with Probation policy.
Pre-Employment Drug Screening:
- Must pass a pre-employment drug screening to ensure a drug-free workplace. A negative drug test result is required.
Driving Skills:
- Must meet the requirements set by our auto insurance carrier.

Work Environment:
The work environment involves interaction with inmates who are about to be released from jail, as well as various community partners such as mental health programs, community health clinics, Collaborative Courts, parole agents, and other relevant stakeholders. Multi-disciplinary team meetings with both Sheriff's Office personnel and community partners might take place within the office premises or other designated locations. Considering the nature of the job, the work environment may involve dealing with sensitive and challenging situations, as it deals with inmates, some of whom may have co-occurring disorders or serve lengthy sentences. The Staff will need to maintain a professional and compassionate approach while working with these individuals.
The work environment may also require the staff to manage an assigned caseload, prepare reports and documentation, and potentially provide court testimony, indicating that there could be office-based tasks as well.
