The Good Samaritan Society is seeking a skilled Care Coordinator to integrate healthcare for clients from pre-admission to post-discharge. As a key member of our team, you will coordinate and sequence client care, enhancing patient/resident flow and promoting interdisciplinary communication.
This role requires independent nursing judgment, knowledge of age-specific structured care methodologies, and the ability to communicate effectively with patients, family members, and healthcare teams. You will monitor, evaluate, and trend patient/resident responses using structured care methodologies, ensuring fiscally responsible practices.
Our ideal candidate possesses a strong foundation in nursing, with a minimum of one to two years of professional experience in a clinic care setting. A Bachelor's degree in nursing is preferred, along with graduate-level education from a nationally accredited nursing program. Multistate licensure is also an asset.
