Our mission is to provide individualized nursing care in the home setting, utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to ensure the best possible outcomes for our residents. We collaborate with other inter-professional colleagues to plan, implement and evaluate care.
As a Clinical Care Leader, you will be responsible for documenting resident history, symptoms, medications and care provided in compliance with applicable standards/regulations. You will utilize technology to enhance quality resident care, to collaborate with inter-professional team members, effectively delegate to and supervise unlicensed personnel, assume overall nursing accountability for a resident assignment and serve in leadership roles on department/organizational tasks forces and committees.
