Job Duties
* Conduct comprehensive assessments of residents' physical, emotional, and social needs
* Develop and implement personalized care plans that address each resident's unique requirements
* Collaborate with other healthcare professionals to ensure seamless coordination of care
* Provide education and support to residents and their families on various aspects of care
Qualifications
To be successful in this role, you will need:
* A current RN license in good standing
* A degree in nursing from an accredited program
* Experience working with older adults and/or individuals with complex medical needs
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
