Job Summary
This is a PRN Registered Nurse position that requires a strong background in long-term care. The successful candidate will be responsible for coordinating care, conducting resident assessments, and providing education and support to residents and their families.
The ideal candidate will have a strong commitment to delivering high-quality care and a passion for working in a collaborative team environment. We require a minimum of 3 months of experience in long-term care, with 6 months preferred.
Responsibilities
- Coordinate care for residents, including developing and implementing individualized care plans
- Conduct resident assessments to identify changing needs and develop strategies for improving care outcomes
- Provide education and support to residents and their families on various topics, including medication management, disease prevention, and healthy lifestyle choices
- Work collaboratively with other members of the healthcare team to ensure seamless delivery of care
- Maintain accurate and timely documentation of resident care and progress
Requirements
- Current RN license with the State Board of Nursing or multistate licensure if working in a Nurse Licensure Compact state
- Minimum of 3 months of experience in long-term care, with 6 months preferred
- Strong commitment to delivering high-quality care and passion for working in a collaborative team environment
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment with multiple priorities
