Job Summary: As a RN Case Manager, Home Health, you will play a vital role in integrating healthcare for clients from pre-admission to post-discharge. This involves coordinating and sequencing the client's care, enhancing patient/resident flow, and promoting interdisciplinary communication for early intervention.
About the Role: In this structured and unstructured health care setting, you will utilize independent nursing judgment to evaluate outcomes based on integration among established clinical, financial, and utilization data. You will have knowledge of age-specific structured care methodologies, such as protocols/integrated clinical pathways/guidelines/standards of care, relating to the overall health care needs of neonatal, pediatric, adolescent, adult, and/or geriatric patients/residents.
Key Responsibilities:
- Coordinate and facilitate care for clients from pre-admission to post-discharge
- Evaluate outcomes based on integration among established clinical, financial, and utilization data
- Utilize age-specific structured care methodologies to address overall health care needs
- Communicate effectively with patients/residents, family members, and other healthcare team members
- Fiscally responsible and able to control variances and duplication
Requirements:
- Bachelor's degree in nursing preferred
- Graduate from a nationally accredited nursing program
- Minimum of one to two years of professional nursing experience in a clinic care setting preferred
- In-depth knowledge of specialty area preferred
- Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring
