**Key Responsibilities:**
- Provide direct patient care, including administering medications, performing treatments, and documenting patient interactions.
- Assess patients' physical and psychosocial needs, identifying areas for improvement and developing strategies for addressing these needs.
- Develop and implement individualized care plans that meet patients' unique needs and preferences.
- Collaborate with other healthcare professionals to ensure comprehensive care and optimal patient outcomes.
**Requirements:**
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse (LPN).
- Currently holds an unencumbered LPN license with the State Nursing board and/or possess multi-state licensure privileges.
- Ability to work effectively in a fast-paced environment, prioritizing multiple tasks and deadlines while maintaining attention to detail.
