About the Job
The Clinical Care Leader (CCL) is a key member of our care team, responsible for providing individualized nursing care to residents in their homes.
This role requires the CCL to utilize the nursing process, including assessment, diagnosis, outcomes/planning, implementation, and evaluation, to ensure that each resident receives the highest level of care possible.
The CCL will collaborate with other inter-professional colleagues to plan, implement, and evaluate care, using technology to enhance quality resident care and collaborate with team members effectively.
Responsibilities
The CCL is responsible for:
- Providing individualized nursing care to residents in their homes.
- Collaborating with other inter-professional colleagues to plan, implement, and evaluate care.
- Documenting resident history, symptoms, medications, and care provided in compliance with applicable standards and regulations.
- Assuming overall nursing accountability for a resident assignment and serving in leadership roles on department/organizational tasks forces and committees.
Requirements
A bachelor's degree in nursing is preferred, and graduate from a nationally accredited nursing program is also preferred.
A minimum of two years' long-term care or geriatric nursing experience is required, with MDS experience being preferred but not required.
The CCL must hold an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
