Nursing Professional Opportunity
The Good Samaritan Society is seeking a qualified RN Case Manager, Long Term Care (LTC) to join our team at GSS MN Luverne. As a member of our care coordination team, you will be responsible for assessing, implementing, and coordinating plans of care for residents in our long-term care community.
Responsibilities and Requirements
- Assess and coordinate plans of care for residents in pertinent areas
- Ensure all residents meet assisted living qualifications as identified in the occupancy agreement, if applicable
- Bachelor's degree in nursing preferred
- Minimum of one to two years of professional nursing experience in a clinic care setting preferred
