Job Summary:
As a Care Coordinator, you will integrate health care for clients from pre-admission to post-discharge. This involves coordinating and sequencing the client's care, enhancing patient/resident flow, and promoting early intervention through interdisciplinary communication.
You will evaluate outcomes based on integration among established clinical, financial, and utilization data. You will work in structured and unstructured health care settings, utilizing independent nursing judgment when integrating health care.
The ideal candidate will have knowledge of age-specific structured care methodologies, such as protocols/integrated clinical pathways/guidelines/standards of care relating to the overall health care needs of neonatal, pediatric, adolescent, adult, and/or geriatric patients/residents.
Responsibilities:
- Coordinate, facilitate, and negotiate with others to provide comprehensive care.
- Monitor, evaluate, and trend patient/resident responses using structured care methodologies.
- Fiscally responsible, controlling variances and duplication.
- Demonstrate independence, assertiveness, and critical thinking when working with patients/residents and co-workers.
Requirements:
- Bachelor's degree in nursing preferred.
- Graduate from a nationally accredited nursing program preferred.
- Minimum of one to two years of professional nursing experience in a clinic care setting preferred.
- In-depth knowledge of specialty area preferred.
- Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
