Salary and Job Description
We are seeking a Clinical Care Leader/MDS Nurse to join our team. As a member of our team, you will be responsible for utilizing the nursing process to provide individualized nursing care in the home setting. You will collaborate with other inter-professional colleagues to plan, implement and evaluate care. You will document resident history, symptoms, medications and care provided in compliance with applicable standards/regulations.
Required Skills and Qualifications
- Effective communication skills to collaborate, negotiate and resolve conflicts, situations, and/or issues with peers, inter-professional colleagues, residents and families.
- Strong clinical reasoning and critical thinking skills to analyze data and appropriately respond to changing situations.
- Bachelor's degree in nursing preferred. Graduate from a nationally accredited nursing program preferred.
- Minimum of two years' long term care or geriatric nursing experience preferred. Minimum Data Set (MDS) experience preferred but not required.
