Job Summary:
We are looking for a licensed Physical Therapist to provide physical therapy programs that help clients recover and improve their mobility, relieve pain, increase strength, and prevent deformities. You will conduct thorough examinations, evaluations, and interventions for patients affected by injury or disease, utilizing evidence-based practices and adapting treatment plans as needed.
Key Responsibilities:
- Plan, organize, and deliver individualized physical therapy programs for patients with varying diagnoses and disorders.
- Conduct comprehensive evaluations, including assessments of patient goals, medical history, and functional limitations.
- Develop and implement therapeutic plans, incorporating current research and best practices, with regular adjustments as necessary.
- Collaborate with healthcare teams to ensure seamless transitions between settings and optimal patient outcomes.
- Document patient information accurately, establishing clear goals and progress toward rehabilitation milestones.
- Educate patients, families, and caregivers on self-exercises and home-based activities to promote continued improvement and adherence to treatment plans.
Requirements:
- Graduate degree in Physical Therapy from an accredited program.
- Current licensure in the state(s) of practice as a Physical Therapist.
- Valid driver's license (when required).
- Basic Life Support (BLS) certification within 60 days of employment.
