Program Overview

The Office of Diversion and Re-Entry (ODR) Program serves former criminal justice-involved individuals by providing reentry support services with the goal of, reducing recidivism, and improving the health outcomes of justice involved populations who have underlying health needs. The holistic and client-centric program assists clients with transitioning out of the justice system with reintegrating into their communities.This population can include individuals with a high emergency room or hospital admission, those who have or are currently experiencing homelessness, those with mental illness and/or substance abuse disorders, or those who may be pregnant.

The Community-Based Intensive Case Management Services Team is responsible for providing resources and supports to improve overall health, social, and economic outcomes of persons served by the program. The CHW will provide intensive case management services to persons who are reintegrating into their communities. The CHW will collaborate directly with the Executive Director, Licensed Clinical Social Worker and staff to ensure that client needs are met with compassion and zeal, through the provision of exceptional case management.

Responsibilities and Duties

·         Provide intensive case management which includes, but is not limited to, screening and assessing      clients; creating, implementing and monitoring individual service plans; crisis intervention; connecting           clients to medical, mental health, employment, housing and other community resources

·         Meets regularly with clients to develop individualized reentry care and life plan goals; encourages progress towards these goals through community resources.

·         Assist clients in obtaining any and all public benefits for which they are eligible

·         Mentor, motivate and empower the client to successfully re-enter the community by establishing a trusting and open relationship with the client

·         Provide health education and counseling around disease management, medication adherence, mental health and SUD care, and self-care

·         Identifies community resources appropriate to meet clients’ needs as identified by reentry care plan

·         Assists clients in navigating health system through appointment accompaniment, transportation        assistance, and coaching

·         Works with community-based reentry programs and services to link clients to housing, employment,   and other social services.

·         Ensure clients are linked to and accessing medical health, mental health, and SUD services as needed, including assisting clients with establishing a medical home and maintaining continuity with       their medical home, if needed

·         Assist clients with maintaining medication and treatment regimens, including accompanying clients to appointments with health, behavioral health and/or other care providers

·         Submit weekly and monthly case management documentation as required

·         Compose and process various types of correspondence to include letters, sensitive and confidential   client information, and other client documentation

·         Travel as required to meet client needs and program guidelines

·         Other duties as assigned

Qualifications

Qualifications and Skills

·         Candidates who have experienced incarceration for a continuous period of at least six months are           encouraged to apply

·         Demonstrated ability to reintegrate into your community following incarceration

·         Ability to establish rapport and build trusting relationships with clients

·         Experience working with diverse clientele who may have a history of extensive legal, medical, mental      health, and/or substance abuse challenges.

·         Ability to independently develop case plans and perform evaluations and assessments.

Preferred Qualifications

·         Degree in Human Services or related field

·         Experience assisting community members in navigating health and social service systems is preferred

Job Type: Full-time

Salary: $32,000.00 to $35,000.00 /year
