Community-Based Palliative Care Nurse Practitioner
Named by the Boston Globe as one of the Top Places to Work in Massachusetts for the last three years, our community based, mission-driven hospice and palliative care organization seeks a Community-Based Palliative Care Nurse Practitioner to join its dynamic team. Travel is required for this position. Care is provided in patient’s private homes and residential facilities throughout Greater Boston and parts of Metrowest.
Job Responsibilities:
The Community-Based Palliative Care Nurse Practitioner provides initial and follow-up palliative care consults to patients in the community and assesses and makes recommendations regarding their goals and plan of care.
Schedule and Weekly Hours:
Monday to Friday from 9 am to 5 pm (40 hours).
This role is also offered for 30 hours or more.
Education, Skills and Experience:
- Master’s degree from an accredited program and current nurse practitioner license.
- Certification as an Advanced Certified Hospice and Palliative Nurse preferred.
- 2 years of experience with at least 1 year of experience in hospice, oncology, and/or palliative care
- Commitment to palliative care philosophy and familiarity with regulatory requirements related to Medicare Part B billing practices.
- Demonstrates outstanding communication skills in working with staff, the medical community, patients and families
- Comfort working with socioculturally diverse patients/families
- Ability to take initiative, work independently, multi-task and manage time well
- Comfortable documenting in an electronic medical record (EMR)
- A professional and caring demeanor
