Position Detail : Full-time, exempt position. Schedule is typically Monday - Friday 8:00am - 4:30pm. Additional hours as needed

Position Summary: The Director of Social Services is responsible for overseeing and managing social work services within a nursing home setting. This includes coordinating and implementing social work programs, assessing residents' psychosocial needs, collaborating with the interdisciplinary teams, and ensuring compliance with regulatory requirements along with state and federal requirements. The role involves supporting residents and their families, addressing mental health concerns, and advocating for residents' well-being.
Essential Job Functions & Responsibilities:
- Oversees and manages social work programs within Good Shepherd Fairview Home
- Conducts evaluations of individuals psychosocial needs as well as completing all necessary evaluations such as BIMS, Trauma Informed Care, PHQ-9, Social Determinants of Health, and CAAs once deemed needed and update the care plan accordingly
- Maintain adequate records and documentation within state and federal regulations
- Maintain and update person centered care plans for residents
- Participate in weekly resident care planning conferences. This includes but is not limited to preparing all necessary care plans, extending invitations to residents and their family/representative and documenting accordingly
- Assist with the admission process and complete all necessary admission evaluations and document accordingly and obtain information for discharge planning goals and/or barriers
- Review eMOLST for each new admission. Also to be reviewed each quarter and as needed and update eMOLST database accordingly. Notify nursing to reflect orders are placed and provider.
- Act as a resident and family contact for resident related grievances, missing property, investigations, and ensure timely follow up to the resident/family and interdisciplinary team
- Facilitate and participate monthly in Resident and Family Council and notify the Executive Director and interdisciplinary team of any concerns
- Participate in necessary meetings as needed such as Leadership, morning report, rehab meeting, and admissions meeting and other necessary meetings as scheduled
- Provide professional work ethics with peers, families, residents, while managing multiple tasks in a calm, patient and supportive manner.
- Oversee Corporate Compliance meeting each quarter
- Participate in Quality Assurance and Performance Improvement Processes and complete necessary QA audits and provide to the Executive Director each month
- Engage in community outreach activities and collaborating with external agencies to enhance support services and/or for discharge planning purposes and person centered care
- Refer residents to social, health and community agencies and complete accurate documentation in residents’ records concerning the results of such referrals
- Follow Residents’ Rights policies at all times
- Consistently work cooperatively with residents, residents’ representatives, facility staff, physicians, consultants and ancillary service providers
- Maintain availability for ongoing contact with residents and families for discharge and transfer planning. Provide counsel to residents and/or families in regard to resident related issues
- Meet with social services staff as needed to address questions or concerns
Qualifications: Organizational and multi-tasking skills. Attention to detail and accuracy. Must be 18 years of age. Ability to read, write and speak the English language. Ability to maintain confidentiality.
Experience: 1-3 years of related work experience as a DSW and/or MSW
