PURPOSE OF POSITION: Collaborates with care team to develop, implement and monitor patient care plans through interdisciplinary team processes. Coordinates plans in conjunction with patients, family members and other necessary entities in an outpatient clinic setting. The clinic care coordinator works directly with providers and patients to assess patient goals, as well as, patient's biophysical, psychosocial, environmental, functional, and financial status. Acts as clinic resource to ensure timely responses to patient and family inquiries; provides guidance and assistance to staff members to facilitate necessary care coordination while maintaining positive patient experiences. Works with medical staff to assists or facilitate timely processing of needed authorizations in order for patients to receive requested and/or necessary care. Coordinates with providers, patients and families to arrange for services which require patient and family decisions and participation. Researches and works to procure scarce services for patients while serving as an advocate on behalf of the patients and family members. Is the primary liaison between providers, community health resources, patients and family members as applicable. Continuously communicates with patients, families, medical staff, caregivers, and other resources ensuring medical needs are met. Facilitates or provides necessary patient and family education on treatment plans, resources etc. Develops and maintains a positive work climate and positive working relationships with patients, families, medical staff and community agencies.
Responsibilities
ESSENTIAL JOB FUNCTIONS: Regularly communicates with providers, patients, families and community resources to coordinate care and needed services. Keeps all members of the care team up to date on progress. Maintains a care plan and documents into patient chart, progress, follow up and other pertinent information necessary to meet care goals.Responds to incoming phone calls and/or messages from patient, providers, insurance companies, pharmacies and others on patients care team to ensure care needs are met. Documents in patients chart. Coordinates needs and authorizations for equipment, medications, supplies and other needs to include but not limited to scheduling appointment, obtaining applicable records pertaining to care plan. Performs patient education as directed by provider.Assists with other administrative duties as applicable to patients care coordination, admin duties could include but are not limited to, answering messages, scheduling appointments etc.Assists with all other duties as directed by manager or supervisor to ensure efficiency and productivity.Floats and helps cover back office needs as directed by manager or supervisor. Helps cover back office staffing shortages. Rooms patients and performs nursing visits i.e blood pressure checks, staple/stitch removal, shots only visit, weight check, etc. as directed by manager or supervisor..Position may require working variable hours including nights and weekends and within various specialties.
Qualifications
QUALIFICATIONS:Education- High school graduate or equivalent preferred.Required: Experienced working in a clinical setting.Current BLS certification or ability to obtain one 30 days of hire.Other- Bilingual and/or English-Spanish speaking preferred. Have the ability to work with a culturally diverse population.
