Responsibilities:
- Conduct thorough assessments to identify patient needs and develop personalized care plans.
- Maintain accurate and detailed patient records, including progress notes and treatment plans.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
- Collaborate with the interdisciplinary team to develop and implement comprehensive care plans.
- Provide emotional support and guidance to patients and families throughout the care journey.
Requirements:
- Current licensure as a Registered Nurse in the state of Massachusetts.
- Minimum one year of experience in hospice, home health, or geriatrics.
- Strong communication and interpersonal skills.
