Nursing Assistant
Provide direct resident care services according to the plan of care prepared by and under direct supervision of the Charge Nurse.
Essential Functions:
- Nutritional Care:
- Ask residents about their favorite food and beverage choices.
- Serve meals and snacks with correct diets in a timely manner.
- Assist or monitor residents with eating.
- Encourage adequate intake as directed.
- Record intake as directed.
- Supply fresh water during each shift.
- Measure weight for residents as directed.
- Report changes in appetite, intake, and ability to eat.
- Continence Care:
- Support use of bedpan, urinal, commode, or toilet as appropriate.
- Follow established bowel and bladder program.
- Collect routine urine, stool, and sputum samples as directed.
- Keep urinary catheter and tubing properly positioned and cleaned.
- Empty and record output for each shift as directed.
- Provide colostomy care as directed and observe for results.
- Record bowel movements and character as directed.
- Keep incontinent residents dry and clean.
- Cleanse after each incontinent voiding or bowel movement.
- Report changes in elimination patterns.
- Mobility Assistance:
- Assist with ambulation as needed.
- Use walkers, canes, wheelchairs, gait belts, and other mechanical aids as per the care plan.
- Transfer residents between beds, wheelchairs, geriatric chairs, or commodes using assistive devices and proper body mechanics.
- Maintain joint functioning for daily activities through positioning.
- Move and reposition residents lying in bed regularly.
- Turn residents using proper techniques and body mechanics.
- Encourage residents to be active.
- Conduct range of motion exercises as directed.
- Assist with rehabilitation and restorative programs.
- Encourage participation in therapeutic activities.
- Assist residents to and from social and therapeutic activities.
- Report changes in mobility patterns or safety.
- Report incidents or injuries immediately.
- Personal Hygiene Cares:
- Assist with bathing, shampooing, hair care, skin care, grooming, and perineal care.
- Assist with dressing and undressing.
- Assist with oral and denture hygiene.
- Assist with nail care.
- Assist with shaving.
- Report changes in ADL abilities or skin integrity.
- Comfort and Quality of Life Needs:
- Embrace Culture Change philosophy and make decisions based on resident preferences.
- Assist with meeting needs for uninterrupted rest and naps.
- Watch for signs of discomfort and report to nurses immediately.
- Report resident complaints of discomfort to nurses immediately.
- Follow resident care plan choices.
- Assist with socialization, exercise, and mental stimulation.
- Maintain a clean, comfortable, and pleasant environment.
- Change bed linens and make beds.
- Tidy resident rooms.
- Observe proper ventilation, temperature, light, and noise control.
- Effective Communication:
- Communicate therapeutically and respectfully with residents.
- Provide compensation for diminished hearing.
- Establish eye contact and speak directly to residents.
- Use low decibel sound levels.
- Decrease extraneous noise during conversations.
- Respond to verbal and non-verbal expressions of needs.
- Report if requests cannot be met.
- Receive shift reports from nurses.
- Communicate professionally between staff for continuity of care and safety.
- Report off to nurses before leaving duty.
- Watch for changes in usual communication patterns and report to nurses immediately.
- Protection:
- Practice basic medical asepsis to prevent infection transfer.
- Position call lights within reach.
- Use protective devices according to the plan of care.
- Observe No Walk By call light policy and respond promptly.
- Participate in Catch a Falling Star Fall Prevention program and other safety measures.
- Know and practice HIPAA privacy regulations.
- Provide compensation for impaired visual and hearing.
- Report potential hazards, incidents, or injuries immediately.
- Observe self-back safety and body mechanics procedures.
- Know and follow each resident's plan of care unless status has changed.
- Seek additional help when warranted.
- Know and practice all established policies and procedures.
- Know and practice all emergency and disaster plans.
- Know and perform job procedures, skills, body mechanics, individual resident care plans, and equipment use prior to performing duties.
- Aware of resident, staff, and public safety always.
- Provide resident protection by using special need safety devices and assisting or supervising smoking.
- Report vulnerable adult issues as mandated reporters.
- Watch for and report changes in physical status, mental status, and behavior immediately.
- Other Skills (with special training):
- Taking vital signs.
- Changing oxygen tanks.
- Documenting care provided, vital signs, intakes and outputs, behaviors, and others as directed.
- Performing other duties assigned by licensed nurses or Household Managers.
