JOB SUMMARY
-
Serves as a liaison between patients, family/caregivers and therapy staff within an outpatient pediatric and adult neurodivergent program setting. Provides coordination of therapeutic and support services to patients/clients and/or family/caregivers who are scheduled for therapy or seeking services. This provider enhances communication and exchange of information among the outpatient care team to assure appropriate and timely utilization and access to internal and external services and advocates for the rights and needs of patients. They will also develop community resource contacts and educate the multi-disciplinary team and patients about such community resources.

ESSENTIAL FUNCTIONS

PROVIDE COMMUNICATION AND COORDINATION OF SERVICES
-
Assesses individual needs of patients and families/caregivers, in order to provide full participation in the plan of care, in collaboration with the treatment team.

Manages therapy schedule/waitlist to maximize patient accessibility to therapeutic services.

Obtains accessory information from family members, clinical staff and other relevant sources of information in order to maximize participation and success in their therapy program.

Coordinates efforts with other treating professionals and agencies.
-
Participates in professional collaborations and/or interdisciplinary treatment teams to maximize progress.

Initiates strategies and resources to enhance overall patient experience, such as transportation, education, and community support.

Establishes rapport with community agencies and support groups to enhance client access.

Coordinates volunteer program.
-
Communicates with physician liaisons, while building relationships with regular referral sources.

Communicates updates and program changes with marketing/communications dept.

Manages equipment needs.

ESTABLISHES AND MAINTAINS COMPASSIONATE ENVIRONMENT

Promotes high-quality customer service via courteous greeting, referring to other members of treatment team as needed.

Provides regular communication to treatment team via documentation and team meetings, as well as ad hoc communication as needed.

Maintains patient and organizational confidentiality standards, in accordance with Good Shepherd policy and ethics code, professional ethics code, and HIPAA standards.

Bridges communication between team members, family, and outside services,

CONTRIBUTES TO TEAM EFFORT

Functions as part of the interdisciplinary rehabilitation team within programmatic areas of responsibility.

Assists with development of support groups, social opportunities, and community events.

Addresses patient safety by identifying and communicating patient safety issues to the team, and patients/family.

Promotes positive work environment.
