JOB SUMMARY
-
Facilitate and monitor the execution of clinical outreach for Good Shepherd Rehabilitation Network. Establish referral base, market referral sources, obtain referrals/admissions and utilize clinical skills to market our network services within the community.

ESSENTIAL FUNCTIONS
-
DEVELOPS REFERRAL BASE
-
Assist with coordinating outreach to all referral sources.
-
Perform market research in order to understand the profile of referral sources and their needs.
-
Analyze the demographic and patient details of potential referral sources to assist in formulating new marketing strategies.
-
Forms strategies based on GSRN growth goals.
-
Work directly with marketing representatives to build and maintain relationships with strategically targeted community physicians, medical groups, community hospitals, and other health systems to expand the referral base for both IP and OP.

Coordinates tours, in-services and lectures for referral outreach.

PROVIDES EDUCATION
-
Identify and meet with potential referral sources.
-
Develop and present educational in-services to potential referral sources including physicians, case managers, outpatient providers and insurance companies.
-
Educate potential patients/families as to the rehabilitation program benefits and services of GSRN.
-
Participate in community events, senior fairs and other promotional opportunities.
-
Offer community presentations and health screenings.
-
Serve as a rehabilitation consultant for referrals.
-
Provide potential patients and referral sources with the most up to date outcome information for GSRN and clearly articulates how GRSN differentiates itself against competitors.

MAINTAIN TRACKING LOGS
-
Tracks visits and calls to potential referral sources and the results of those calls.
-
Generate monthly and quarterly reprts on all outreach from tracking logs.
-
Create and administer new fiscal year tracking logs.
-

IDENTIFY APPROPRIATE LEVEL OF CARE
-
Serve as a resource for all referral sources to navigate access to Good Shepherd’s continuum of care.
-
Work with appropriate clinical teams to determine that services are appropriate, reasonable and necessary.

Provide recommendations as to appropriate level of care

MAINTAINS QUALITY SERVICE
-
Serves as a conduit for feedback to referral sources on individual patient outcomes.
-
Resolves referral source complaints by investigating problems, developing solutions, preparing reports and making recommendations for any necessary change.
-
Develop referral source satisfaction screening to identify opportunities for improvement.
-

CONTRIBUTES TO TEAM EFFORT
-
Assists teammates as needed and works collaboratively with Associates from all clinical areas.Advises the marketing and strategic planning team on any and all new opportunities in current market.

ONGOING CLINICAL EDUCATION

Maintain professional and technical knowledge by attending educational seminars, reading professional publications, establishing personal networks and participating in professional organizations.

TRAVELS EXTENSIVELY AND WORKS INDEPENDENTLY
-
Works independently both on and off site and travels as necessary to create strategic outreach areas in geographic targeted areas.

Works independently to identify new opportunities to promote GSRN services in new targeted populations and areasRepresents GSRN at healthcare and other professional venues.

Provides weekly update reports and monthly scorecard reports on activity and targets to direct supervisor.

Education

Bachelor's Degree in clinical discipline required

Work Experience

5-7 years of rehabilitation or home care experience required

1-2 years of experience networking, including corporate presentation experience required
