**Job Description:**
We are seeking a skilled Physical Therapist to join our team. As a Physical Therapist at Good Shepherd Rehab, you will be responsible for:
* Assessing patient condition and developing holistic care plans
* Planning and implementing rehabilitative care, collaborating with healthcare teams and patients
* Providing educational information and answering questions for patients and families
* Monitoring care delivery, identifying process improvements and deviations from the care plan
* Assuring quality of care, measuring outcomes and making adjustments as needed
* Protecting patients, staff, and self from infection and maintaining a safe working environment
* Respecting patient rights, providing emotional and psycho-social support
* Maintaining professional knowledge through education and networking
* Contributing to the team effort, volunteering for community events and mentoring new associates
**Required Skills and Qualifications:**
To be successful in this role, you must have:
* A Bachelor's Degree in Physical Therapy
* Previous home care or ALF experience preferred
* Physical Therapist license required
* Basic Life Support / CPR certification required
