Reports To: CBRF RN Director
Reporting Authority: The LPN is delegated the authority to carry out the position responsibilities and is accountable for assigned duties.
Job Objectives: The LPN provides a variety of services to CBRF Residents and staff. Assists in ensuring regulatory compliance, quality standards and safety to both our employees and those we serve.
Essential Job Functions:
- Provides case management services to CBRF Residents. Assists CBRF RN Director in providing health monitoring of Residents per policies and procedures.
- Coordinates day-to-day appointments for Residents. Sets up outside transportation and/or provides transportation with GSS vehicle to and from appointments.
- Monitors and maintains up to date electronic and paper health records reflecting Residents' physical and mental health conditions.
- Reports significant changes in Residents' conditions to his or her physician in a timely manner. Collaborates with CBRF RN Director. Attends daily staff huddles.
- Follow-up on lab tests, treatments and procedures with physicians, clinics and nurses. Maintains lab flow sheets and current orders. Manages incoming orders via fax and or in writing. Communicates medication orders with pharmacy.
- Performs medication administration.
- Communicates effectively with tenants and families over the phone, in person and via email.
- LPN will delegate tasks commensurate with training. LPN provides supervision, observation and monitoring of staff as needed.
- Serves as a community liaison offering outreach services. Provides monthly education for Resident meetings.
- Oversees CBRF Falls program.
- Performs other duties as assigned.
- Covers staff call-ins as needed. This can include off-shift, weekend and holiday hours. The CBRF operates 24 hours a day, 7 days a week. At times, the needs of the facility come up with very short notice.
- On call for staff issues and scheduling needs. On-call rotation is shared between CBRF RN Director, CBRF LPN, Meadow Wood RN Director and Meadow Wood LPN.
Qualifications:
- Must be a Licensed Practical Nurse in the State of Wisconsin.
- Must possess a high degree of interpersonal relations skills and capability of relating to a variety of people and personalities. Considerable initiative, judgment, and leadership skills are also required. Prefer experience in working with elderly.
- Assist with personnel management when RN Director is not present.
- Must possess strong organizational and time-management skills.
- Must possess computer skills for charting and medication management.
- Must possess a current and unrestricted driver's license.
