Locations:              NYC/City Wide (Mobile Van)

Hours:                    21 hours/Week, Exempt (Flexibility Day/Evening Shift)

Salary:                    $58,000 - $72,727 (Salary Range is based on type of licensure; relevant work experience and more) 

 

Job Summary

As a member of a multidisciplinary team, the Street Outreach Clinical Social Worker will work with participants ages 18-24 experiencing homelessness in the community and provide non-judgmental, harmreduction focused onsite clinical support.  The Clinical Social Worker will participate in the Sanctuary Model of care to insure trauma-sensitivity to all residents.

Major Duties:

  • Provides onsite individual therapy, using agreed-upon models.
  • Available to go into the community with the peer outreach workers as scheduled to engage and offer clinical support to participants.
  • Functions as a member of the multidisciplinary team. Participates in Case Review Meetings and other related meetings.
  • In collaboration with other team members, arranges for wrap-around services in the community the program participant/s is regularly known to be active in. This can be in any of the five boroughs of NYC.
  • Participate in community events where the team will be posted offering clinical support and education/resources.
  • Attend and participate in GSS meetings and trainings as directed, as well as any external meetings/training.
  • Responsible for documenting services/contacts on the GSS program database and other such city databases, as well as additional specific documentation in a timely manner.
  • Supports the Sanctuary model of trauma informed care.
  • Effectively uses and practices the Sanctuary Model tools: safety plans, community meetings, psycho-educational groups, team meetings, red flag reviews, treatment planning conferences (or similar), and self-care in the program to create a non-violent and trauma-sensitive culture.
  • Promotes and models the seven Sanctuary Model commitments to create a non-violent and trauma-sensitive culture.
  • Perform other duties as required.

 

Qualifications:

  • Master’s degree in Social Work and licensure (LCSW) or LMHC required.
  • Clinical work with youth preferred; including crisis management.
  • Must have a clear understanding of the needs of young adults experiencing homelessness and a sensitivity to ethnic and cultural differences, and an understanding of the needs of the LGBTQ community.
  • Must have the ability to communicate with staff on all levels.
  • Demonstrated competence in verbal, written, organizational, and prioritization skills.
  • Capability to transition easily between working independently with a high degree of self-initiative and contributing as part of a team.
  • Excellent crisis intervention, advocacy and problem solving skills.
  • Must have a commitment to work from a strength-based, youth development and trauma-informed perspective.
