Hours : 35 hours, exempt
Job Summary of Social Workers : The Brooklyn Transitions Program is a specialized borough-wide prevention program for families challenged by substance use and / or mental health issues. The Social Worker / Case Worker provides home-based, intensive, strength-based, comprehensive services using the Solution-Based Casework (SBC) framework.
