Young Adult Case Manager - PART TIME
$750 Sign-On Bonus (Restrictions Apply)
Summary
Provide case management and support services to assist participants in maintaining employment and achieving a better quality of life.
Essential Duties & Responsibilities
Develop a curriculum plan for service delivery.
Provide participants with training, job-readiness, and social skills.
Act as a resource and guide, serve as a role model and mentor to participants.
Assist participants of Goodwill Easter Seals Miami Valley services to maintain employment and achieve a better quality of life.
Assess program referrals and establish candidacy.
Coordinate participant referral and involvement in the program with referring Goodwill staff.
Develop and coordinate a written plan with all participants.
Maintain activity report and case records on all participants in each program.
Link participants to community resources as needed.
Advocate for and represent participants with employers and community organizations as needed.
Provide job development and retention services for participants as needed.
Maintain contact with participants and the participant’s employer on a prescribed basis, including face-to-face meetings.
Independently work non-traditional hours in order to provide the support needed for the participant to maintain employment.
Refer and coordinate internal referrals to Goodwill Easterseals Miami Valley services when an outside funding/referral source is not available.
Provide the Quality Assurance department with the data needed to track outcome measures.
Comply with all Agency policies, procedures and safety standards throughout work areas and maintain same to meet CARF, HIPAA, Department of Labor and other regulatory standards.
Other duties may be assigned.

Education Requirements
Bachelor’s degree in a Human Service related field, have the ability to demonstrate good written and oral communication skills, as well a 2 years’ experience in one of the following areas: vocational assessment; working with more than one disability population; job placement skills; and/or case management experience preferred.
Knowledge of community resources and a demonstrated understanding of employer needs preferred.
Experience may be considered in lieu of a degree.
