Job Objective:
Nurse-Family Partnership (NFP), a maternal-child, home-visiting program, strives to provide support and education by partnering a highly skilled registered nurse with an at-risk first-time pregnant mother through pregnancy. Following delivery and throughout the child’s first two years of life, the registered nurse supports the new mother and her family as a mentor to promote healthy child growth and development, help reduce child abuse and/or neglect and support them in reaching their goals. Nurse Family Partnership, implemented by Goodwill Industries of Michiana, Inc., (GIM) across 15 counties in Northern Indiana, seeks to address the serious impacts associated with generational poverty and racial disparity that directly contribute to infant and maternal mortality and an individual person’s future success and opportunity. Building on this concept, the position of the Outreach Coordinator strives to create collaborations and partnerships within the communities they support to produce permanent and potential referral sources, build a community of support for the program and its participants, and identify creative opportunities for marketing. Acting as an intermediary among community organizations and services, the Outreach Coordinator will work in collaboration with the NFP teams to conduct outreach activities in designated markets to increase demand and referrals to the NFP program. This position reports to the Director of NFP who evaluates performance annually.

Essential Job Functions:
- Build and maintain relationships with community organizations and referral partners by conducting outreach, making visits, and participating in events to promote NFP and Goodwill programs.
- Collaborate with leadership and stakeholders to grow and sustain a diverse and engaged Community Advisory Board.
- Conduct outreach efforts, including presenting to community groups and healthcare providers, distributing materials, and staffing events to drive referrals and engagement.
- Research and share community resources for NFP families by creating and updating resource maps for the service area.
- Record and analyze outreach efforts, using data to track progress and guide growth in referrals.
- Support Nurse Home Visitors by attending team meetings to identify family needs and connect them with community resources.
- Contribute to the development and implementation of continuous quality improvement (CQI) projects, occasionally taking a leadership role, to enhance operational efficiency and achieve evidence-based outcomes.
- Review outreach progress with the Director quarterly and actively contribute to team meetings, reflective supervision, and quality improvement initiatives.
- Ensure confidentiality and compliance with HIPAA regulations.
- Other duties as assigned by supervisor.
Qualifications
Required Education and Skills:
- Bachelor’s degree in social services or a related field; an associate degree with relevant work experience will also be considered.
- Knowledge of community resources is required.
- Two or more years of related work experience, such as in non-profits, outreach, or roles directly engaging with community groups.
- Experience in urban settings, diverse environments, or communities facing extreme poverty is a plus, as is lived experience.
- Excellent interpersonal and communication skills, with proven ability to collaborate effectively with community and healthcare organizations.
- Strong organizational skills and experience in project development, coupled with the ability to work independently and stay self-motivated with minimal direction.
- Positive, forward-thinking attitude with a willingness to collaborate, strong leadership abilities, and a personable, engaging demeanor.
- Creative and innovative mindset, with a genuine curiosity and eagerness to learn new skills.
- Tech-savvy, including proficiency in navigating social media platforms.
- Ability to pass a criminal background check and drug screen.
- Must have reliable transportation, a current driver's license, and provide proof of insurance to travel to different job sites.
Qualifications:
- Authorized to work in the United States.
- Flexible availability, including some evenings and weekends.
- Ability to travel to various locations, including client homes, satellite offices, community partner sites, and support service locations.
- Able to frequently bend, twist, reach above shoulder level, push and pull, crouch or stoop, kneel, repetitively use feet, and use head and neck in a twisting or static motion and to look up and/or down, and lift and/or move up to 50 lbs.
