POSITION SUMMARY
To provide case management and coordinated services to trainees enrolled in employment and training programs.

EDUCATION:
Bachelor’s degree required in Human Services, Social Work, Psychology, Sociology, or Rehabilitation Counseling or in a related human services field. May be required to obtain Career Development Facilitator certification.
EXPERIENCE:
Experience working with state and local human service agencies and nonprofit organizations serving persons with complex, multiple barriers or needs and coordination of services among multiple entities for personal and workforce development.
KNOWLEDGE/ABILITIES:
Knowledge of case management principles, special populations, regulations and legal issues, vocational counseling and service options and supports. Documented skill in areas of written and verbal communication, group dynamics, service plan development and implementation, advocacy, human relations and time management. Capability to motivate others toward achieving measurable goals. Ability to work in a variety of settings with culturally-diverse families and communities with the ability to be culturally sensitive and appropriate. Ability to effectively address negative behavioral situations or challenges; resolve conflict with diverse populations and situations.
PHYSICAL DEMANDS:
Able to stand or sit periodically for the required work schedule.
MENTAL DEMANDS:
Required to remain objective and to respond appropriately to customers. Must be able to complete and prioritize multiple assignments. Must be proficient with Microsoft Word, Excel, PowerPoint, Outlook and web-based programs. Proficiency with utilizing the Internet. Ability to use and operate a calculator, copier, scanner, fax, printing equipment, multi-line phone systems and other office machines.
WORKING CONDITIONS:
Interactive office position with staff, trainee, and community partners.
OTHER REQUIREMENTS:
Must possess valid driver’s license and access to reliable transportation for work. Annual criminal background check may be required of employees serving trainees within behavioral health services programs.

Position Duties:
- Provide holistic and comprehensive case management services to trainees including: intake assessment, orientation, goal setting, crisis intervention, barrier removal, development of Individual Service Strategy/Comprehensive Assessment, progress monitoring toward goal attainment, and advocacy eligibility for support or program services.
- Provide career assessments including various forms of testing, counseling to support trainee’s vocational/employment goals and facilitate employability workshops.
- Coordinate services with mandated community partners to support trainees in removal of barriers to employment.
- Conduct background checks on prospective trainees as appropriate.
- Interact with trainees in person or through the use of technology to perform eligibility determination, program intake, entrance/exit interview, orientation sessions (individual or group), counseling, intervention (behavioral/crisis), progress monitoring and evaluation, and follow-up and execute necessary tasks at each stage of interaction to ensure trainee’s progression.
- Coordinate and integrate services with other appropriate service organizations to support trainees’ goals, objectives and quality outcomes achievement. Create training plans and work-site agreements for Goodwill customers with their employers
- Manage case records in accordance with Federal, State and Local policies and procedures. Utilize mandated web-based case management programs as required by Federal, State and Local policies and procedures. Adhere to CARF accreditation standards, funding source requirements and audit compliance standards for service delivery.
- Participate in regular meetings, trainee program service planning and professional development trainings required and/or conducted by Michigan Works! agency or other designated entity, including workshops, seminars and conferences.
- Provide on-site monitoring of transitional and community work experience to include trainee time sheet collection and review, regular site visits with trainee and work site supervisor for performance evaluation update, and participation in trainee orientation as needed. Trainee monitoring will also include apprenticeships, on-the-job training and vocational skill development activities. Record trainee progress toward goal attainment in agency and funding database system and trainee manual file.
- Provide regular and timely input of detailed trainee case notes documentation into electronic case management system for persons served for an accurate account of person’s service progress, incidents, intervention, and other significant service related information utilizing agency prescribed method of case notation
- Ensure compliance with established internal quality control procedures for records maintenance, reports, service, operating practices, and documentation in preparation for internal and external auditing, accreditation and other appropriate regulatory bodies.
- Conduct community outreach to develop referrals and relationships for workforce development referrals, i.e. presentations at local libraries, adult education sites, community resource events, community partner agencies.
- Perform other reasonable assignments as required.
