POSITION DESCRIPTION:
TITLE OF POSITION: Workforce Training /CCMEP Case Manager

FUNCTION: To provide Case Management and CCMEP services to consumers referred by Dept of Job and Family Services
ESSENTIAL FUNCTIONS:
1. Case Management and assessment of consumers assigned by Case Management Supervisor. This includes CCMEP.
2. May provide on-site case management services throughout Wayne County Schools
3. Maintains complete records on all consumers receiving services as assigned
4. Performs intake for consumers
5. Meets with consumers on a weekly basis to review progress on service plans and updates progress in the Consumer Database
6. Attends meetings and case staffings as required for each consumer
7. Initiates, maintains and documents service provided for consumers and updates in the consumer database
8. Coordinates daily consumer schedules
9. Prepares required reports on all consumers and trainees
10. Provides progress reports for the Case Management Supervisor on a weekly basis for everyone served.
11. Works directly with consumers and trainees in all phases of the rehabilitation process
12. Provides retention services to consumers
13. May function as a driver when needed
14. Performs other duties as assigned by the Case Management Supervisor and the Mission Services Director
REGULAR DUTIES:
1. Maintains a good working relationship and provide excellent customer service to all consumers, staff, customers and supervisors
2. Has respect for the feelings of others, is courteous and possesses the ability to build effective internal and external relationships
3. Complies with all established work rules, policies, procedures and safety rules
4. Represents the organization in a professional and courteous manner
5. Is able to educate stakeholders and community about the entire agency
REQUIREMENTS:
Positions Supervised: None
Education: College degree in related Human Service field or High preferred, but not required - School diploma or related work experience can replace degree
Experience: 3 years experience in related field preferred
Certification: CPR/First Aid - can obtain upon hire
Other Requirements:
License Check - valid driver’s license and proof of insurability or is able to provide transportation
Background Checks - The offer of employment will be based on the outcome of the background check
Drug and Alcohol Screen - Must pass a drug and alcohol screen prior to employment Subject to random testing
