Position Title : Behavioral Health Care Manager
Location : New Castle County
Schedule : Monday – Friday 37.50 Hours
Pay Rate : $23.50
Job Description :
The individual will work as a part of an interdisciplinary team in a PROMISE program serving individuals with severe and persistent mental illness including those with co-occurring disorders. You will act as a care manager in performing assessments, helping develop individualized recovery plans, determining appropriate levels of care, placing individuals in those levels of care and ensuring that care is delivered according to standards. Other duties will include visiting beneficiaries in the community, charting, and acting as a liaison with a large network of community providers.
Must have a valid drivers license and be able to operate a vehicle.
