- Maintain detailed case notes and documentation to track client progress
- Participate in multidisciplinary team meetings to discuss client cases and coordinate care
- Bachelor's degree in social work, psychology, or a related field
- Minimum 2 years of experience in case management or a similar role

- Certification in case management or a related field is preferred
